Please use this enrolment form for all classes: Gravity Dance, Fame and Recharge Non-Performance.  It is important that the age and date of birth is entered correctly for all Gravity Dance and Fame students.  

Adult dancers over 30 who do not wish to disclose their age on their enrolment form, please use NA.  Ballroom and Latin American dance couples need an enrolment form for each dancer but may choose to have the same Primary Contact for both forms. 

Invoices will be sent to the Primary Contact unless requested otherwise.
